                    DS-11:  Online Application for Passport


    You must use this form IF:

  1. You have never had a U.S. passport
  2. You are under age 16
  3. Your most recent passport was issued more than 15 years ago
  4. Your most recent valid passport was lost or stolen

    -OR-

    If you are applying because your passport was lost or stolen, you will also need to submit Form DSP-
    64 (listed below.) Please follow the instructions on pages 1-2 of the application. Print and complete
    page 3, and print and read page 4 carefully before appearing with pages 3 and 4 to apply for your
    passport.

                   DS-82:  Online Application For Passport By Mail (For Renewals Only)


    This form should be used for renewing a previously issued US passport by mail  OR in person at a
    Regional Passport Agency if you are traveling internationally, or need your passport for foreign visas
    within 14 days.

    There are 4 criteria for using this form, see if you are eligible to apply with a DS-82:

  1. I can submit my most recent passport AND
  2. I was at least 16 years old when my most recent passport was issued AND
  3. I was issued my most recent passport in the last 15 years AND
  4. I use the same name as on my most recent passport or since my passport was issued, I have
    had my named changed by marriage or court order and can submit proper documentation to
    reflect my name-change.

    If you answered NO to any of the 4 statements above…STOP!
    You cannot use Form DS-82! You must apply on a DS-11.

    Note: Page 1 of the DS-82 gives phone numbers to call forinquiries. If you have a question about the
    application process not answered by our website or want to know the status of a pending application,

    The NPIT is currently experiencing a very high volume of calls.  Please expect delays if you wish to
    speak to a customer service representative.   You may have to call several times before reaching a
    representative.  However, please note that representatives are available from 6:00 a.m. to 12 midnight,
    ET, Monday-Friday, except federal holidays.  Representatives are also available for limited weekend
    hours for emergency situations in which travelers are leaving in 7 days or less.

    Correcting or Changing Information in Your Passport

    The forms below should be used for amending a previously issued US passport by mail at anytime OR
    in person at a Regional Passport Agency if you are traveling internationally, or need your passport for
    foreign visas within 14 days. You may also be able to use Forms DS-82 and DS-11, please review the
    chart (link above) carefully to make sure that you take the appropriate action.

    The forms may be used if:

  1. Find a printing error in your passport;
  2. When there is a name change after your passport was printed;
  3. To extend the validity of certain previously limited passports; or
  4. To add visa pages.

                    DS-64:  Statement Regarding Lost Or Stolen Passport


    This form should be submitted along with the DSP-11 Application for Passport and brought with you
    when you go in person to apply for a new passport if your previous passport has been lost or stolen.

    Passports for Minors under the Age of 14

    For Minors under the age 14 you must establish proof of citizenship AND proof of relationship.  
    Previous U.S. passports are not acceptable as proof of relationship to the applying parent(s)/guardian
    (s).

    To prove the relationship you will need to submit one of the following:

  • Certified U.S. birth certificate (with parents' names); or
  • Certified Foreign Birth Certificate (with parents' names and translation, if necessary); or
  • Report of Birth Abroad (Form FS-240) (with parents' names); or
  • Certification of Birth Abroad ( Form DS-1350) (with parents' names); or
  • Adoption Decree ( with adopting parents' names); or
  • Court Order Establishing Custody; or
  • Court Order Establishing Guardianship.

    Each parent or guardian must also provide parental identification by submitting one of the following:

  • Valid Drivers License
  • Valid Official U.S. Military ID
  • Valid U.S. Gov't ID
  • Valid U.S. or Foreign Passport with recognizable photo
  • Naturalization/Citizenship Certificate from USCIS with recognizable photo
  • Alien Resident Card from USCIS

    NOTE: Your Social Security Card does NOT prove your identity .
